“IUI is, in fact, one of the primary procedures we opt for couples having a tough time conceiving where the cause of infertility is uncertain. In theory, it is a simple procedure where we directly place the sperm as near as possible to the egg, essentially removing all the hurdles for the sperm to reach the egg for fertilization. The fertile period is identified, the sperm is collected, purified and is injected into the uterus. IUI is less invasive and also less expensive.” says Dr. Swetha Thumula.
Intra Uterine Insemination is reported to ramp up the pregnancy chances by more than 20% and is rated as highly effective in treating the pregnancy problems caused by lower sperm quantity and quality. The chances of success can be further increased when this insemination procedure is assisted with Ovulation induction.
However, IUI is recommended only if the semen analysis of the man turns out is normal and fallopian tubes of the woman are healthy to take help from the insemination. Women with fallopian tube problems, PCOS or endometriosis are not right candidates for IUI and their infertility problems can be solved with the help of In Vitro Fertilisation procedure.
IVF – In Vitro Fertilisation:
While Infertility caused by poor sperm parameters can be combated by IUI, there are certain complex causes of infertility (PCOS, Fallopian blockage, endometriosis etc.) that need to be treated with a more comprehensive procedure like In Vitro Fertilisation.
“Every woman’s ovaries activate hundreds of eggs but only one stays on to mature for fertilization. In the IVF procedure, this number is boosted and ovaries are stimulated, the matured eggs are retrieved and stored. The motile sperm is extracted from the man and is then combined with the retrieved eggs for fertilization. Fertilized eggs turned embryos are incubated and implanted into the uterus for pregnancy” explains Dr. Swetha Thumula.

ICSI – Intracytoplasmic Sperm Injection:
The conception problems caused by male infertility problems like lower sperm count and poor motility can be solved by Intra Uterine Insemination. Infertility caused by complex female problems can also be bypassed by resorting to IVF. But there can be cases where both male and female fertility issues can dampen the chances of conception even after opting for fertility treatments.
Take IVF for example, the eggs are meticulously retrieved and combined with sperm under laboratory conditions and waited until the natural fertilization takes place. In essence, the fertilization here is still dependent on the sperm count and motility which can be fine for a young couple with potent sperm. But in the case of couples where the number of healthy sperm are limited, the chances of achieving successful fertilization become meagre with conventional IVF.
What if we can select the most potent sperm of the lot and inject it into the egg? And that is what Intracytoplasmic Sperm Injection (ICSI) is all about.
“With ICSI, a single potent sperm is directly injected into the retrieved egg making it easier for the fertilization. These eggs are incubated for a period of at least 18 hours after which the fertilization will have to confirmed by the embryologists” says Dr. Swetha.
